To give you the care you need we keep information about you limited to staff involved in your care and treatment including any illnesses, tests, prescriptions and other treatment you have had. For example, when your GP refers you to a specialist at the hospital we send relevant details about you in the referral letter. A contract exists between the partnership of Dr Roberts, Mr Eddy, Dr O’Sullivan, Dr Melhuish, Dr Hughes, Dr Hattersley, Dr Cook and NHS England for the provision of primary care general medical services to patients within the practice area.
